On your child's first day, you will need to bring the required forms completed, including a copy of your child's shot record.  The "Medical Examination Report" form may be returned within 30 days of the first day of enrollment. This form must be signed by your child's doctor.  In addition, if your child has special needs, the "Individual Child Care Plan" must also be completed by a physician.
